New Passport Application

- Aadhar Card
- Proof of Address (Electricity/Water Bill or Aadhar)
- Proof of Date of Birth (Birth Certificate or Matriculation Certificate,)
- Contact Details such as Email and phone number.

Application Form / Name Change Deed – Duly filled and signed.
Affidavit for Name Change – On stamp paper, notarized by a Notary/Oath Commissioner.
Identity Proof (any one or more):
- Aadhaar Card, PAN CARD,Voter ID,Passport,
- Driving License

The following are needed:

Application Form – Duly filled and signed (as per Gazette office format).
Affidavit – Notarized affidavit mentioning old name, new name, reason for change, and personal details.
Identity Proof – Self-attested copy of Aadhaar Card / PAN Card / Passport / Voter ID.
Address Proof – Ration Card / Electricity Bill / Driving License / Aadhaar Card showing current address.
Photographs – Recent passport-size photographs (usually 2–3 copies).
Supporting Documents – Educational certificates, birth certificate, or any document showing old name (if required).
Marriage Certificate / Divorce Decree – If name change is due to marriage or divorce (for women applicants).
Prescribed Fee – Demand Draft / IPO (Indian Postal Order) / Online payment receipt as per Gazette department.
Request Letter / Covering Letter – Application addressed to the Controller of Publications, Department of Publication (Govt. of India) or State Gazette office.
Cutting/Copy of Newspaper Advertisement – Sometimes required (if Gazette office insists).
